A blank page usually means a php error (check your php and web server logs). Also check
permissions on the extension software and make sure your web service can execute.

Oh sorry, I forgot about the collapsing feature.
You may need also some MediaWiki:Common.js,

or, its good to take from the source,

But I'm not certain at the moment.
I would feel better if this discussion were on-list, so others could
contribute and benefit.
