I have run across a vbulletin forum inside the mediawiki skin also. I can't seem to find it again, but I know that it has the mediawiki sidebar left of the forums, which is my main concern. Google is not my friend today.

Well, apart from looking rather broken in my browser, that site appears to have used a CMS called Mambo which incorporates forums and a wiki - so it's not so much a forum within the wiki, as a forum and a wiki both within something else. Whether MediaWiki is something that comes with Mambo normally, or they've hacked it in there, I can't make out - there doesn't seem to be a straight-forward feature list on the mambo homepage - but basically, they've skinned both so that they look like they go together.

Well, reproducing that ought to be pretty easy in any reasonably skinnable forum software - the sidebar is only a bunch of links, after all, and doesn't need to actually touch the wiki database. And making them visually match is just a case of playing with CSS for long enough.
The hard part is if you want to do things like use the users from one database for both pieces of software - the mailing list archives should reveal lots of people who've at least attempted this... (use "site:mail.wikipedia.org" in a Google/Yahoo/whatever query)
