I'm a week slow in responding to this (I was out of town)...
The only 2 extensions I have running are:
navigation_en.php
DynamicPageList.php
And this is an internally only wiki...no way to access it from outside.

using, and do you have any extensions installed?

Most of the time when this happens it means you have trailing newlines
at
the end of an extension file.
At some point this will be auto-detected by MediaWiki - in the
meantime,

It appears Mediawiki is putting in a blank line above the "<?xml
version="1.0" encoding="utf-8"?>" line, when that line should
be
first.
Any ideas on how I can get Mediawiki to change that behavior?
