Well, the filename doesn't ring any bells for me...
Could you describe the exact steps you followed to install? I should
point out that MediaWiki 1.1.0 doesn't have a web interface to the
installer; install.php must be run from the command line, and is very
finicky even then.
I'd highly recommend you give 1.2.0rc2 a try: it's got a new web-based
installation which should be a little more straightforward (go to
config/index.php).

What is the minimum disk space requirement to install and run MediaWiki? I attempted to install it on a VDS Linux/Apache server hosted by Jumpline with 500mb of space and at install, it returned an error message and it also seems to have used roughly 1500mb mainly in a tmp (500mb) file and in a /proc/kcore (1,000mb) file before giving up. I deleted the tmp which seemed to have been created by the file manager, but I have no idea what to do with the /proc/kcore file.
Since the first release candidate, an in-place web-based install script
has been added. While still a bit experimental and incomplete, it makes
it much easier to get a new wiki started, particularly where shell
access isn't available for the old command-line installer.
*NOTE:* The web install script doesn't yet support updating old
databases or other maintenance procedures. It can create a new
database, or write a basic LocalSettings.php to access an existing
(up-to-date) database without altering it. The new installer is still
experimental and may fail mysteriously; we'd love to hear from you, if
you do or don't get it to work.

MediaWiki 1.2.0 includes improved inline image and thumbnailing
support, smoother account management, and a number of interface tweaks
as well as numerous bug fixes and backend features (squid cache
purging, authenticated SMTP, tighter upload security, PHP5
compatibility). Also fixes an incompatibility with MySQL 3.2.x in the
default install that cropped up in 1.1.

I've got a user who's forgotten his password and needs to get it back.
Is there an easy way that I can reset the password field for him, or
find out the password and send it to him? I've tried looking in
phpMyAdmin, and all I see are the blob and tinyblob fields, and I don't
know how to interpret those.

How can I change the design of MediaWiki?
It seems that it has something to do with the css files in /styles/ and the "Skin*.php" files. Which of the .css files are used and how do the Skin*.php files work?

There've been lots of complaints about the MediaWiki installation
script over time, and with good reason! We don't actually use it for
Wikipedia since it doesn't fit our needs (centralized repository of
scripts for over a hundred wikis with near-identical config), nor does
it fit the needs of a lot of people trying to set up their own wikis,
who often don't have shell access to their web servers or root access
to the database.
I've checked in the beginnings of a new install script that doesn't
require shell access, root database access, or a separate place to put
the source files and the installation. You can just unzip the archive,
copy the directory tree to your web space, and run the configuration
script through the web like with any civilized PHP app with relatively
minimal fiddling.
At the moment it can create a configuration file that works with an
existing database, but doesn't yet do the database setup. I'll try to
get that merged in from the old command-line install script tonight or
tomorrow and release 1.2.0rc2 with it included.
(Tech note: we need to be sure that the maintenance scripts aren't
dangerous to have sitting around.)
