[Mediawiki-l] Adding Get Satisfaction widget to Mediawiki

I am trying to add the Get Satisfaction widget to Mediawiki. I assume there are two routes - one directly in Monobook.php and another as an extension. I'm actually surprised there isn't an extension for this, but could anyone help me out on what to do? Thanks!
