Another question: using the ALTER TABLE command, how will that deal
with existing case-different duplicates? Say we have Apple and ApPLe
- will that prevent the Alter from succeeding?

I have been looking into this for a year.
In Unicode-based schema, just use utf8 character set and
utf8_unicode_ci collation - here you go, case insensitive. This also
maps accents to base latin forms, and similar mappings are done.
You can use 'ALTER TABLE ... CONVERT' syntax to convert your tables.
