You should also check with Spam clearhouse services like Spamhaus and see if you've been listed as a spam site. This can happen if the mailer is set up incorrectly. Unfortunately I can't find my notes on what I did to fix it, but I think it has to do with whether a reply to the sending address gets bounced. After changing our configs, we had to go and request delisting from the spam blockers.

I installed mediawiki using apt-get in Debian Etch. When I click the "confirm email" as a wiki user, the confirmation mail gets tagged as spam in both gmail and hotmail accounts.
Here is a copy of my mail header and mail body:

Someone, probably you from IP address 213.112.81.182, has registered an account "Tommy" with this e-mail address on Neo101wiki.
To confirm that this account really does belong to you and activate e-mail features on Neo101wiki, open this link in your browser:
http://neo101.org/mediawiki/index.php/Special:Confirmemail/ 8536ac84c819099d32acdfe4a30936a2
If this is *not* you, don't follow the link. This confirmation code will expire at 2007-07-26 10:31:53.

How did you solve this problem? I would give a bad first impression if I were to ask my users to look for the confirmation mail in their spam-box.
P.S. I added the "NOSPAM" textstring by hand in this email.
