The database connection details supplied were incorrect. The MediaWiki
installer requires one set to be entered, with the second set being
optional:
The first database username and password is that of the MySQL account
used to connect to the database on a routine basis to perform standard
read and write operations. These account details will be written into
the LocalSettings.php file, and should point to an account which has
*at least* SELECT, INSERT, UPDATE and DELETE permissions on the
database specified for installation.
If the account details provided refer to an account which does not
have permission to create the database (if needed), or to create and
alter tables within the database, then you need to check the box
marked "use a superuser account" and provide details of another
account, such as the MySQL root user, which does have these
permissions.
The most common cause for an "access denied" message is an incorrect
password, so that would be the first thing to check; it's
case-sensitive, as usual. The default MySQL root password is blank.
