You cannot add forms, javascript and php code directly into a wiki page. For this, you should probably find another CMS.
Hello!! What does CMS mean?? Are you sure that doesn't exist any method to add php code or cgi ... in wiki pages?? Thanks!!

Sure. If you want to add php code, you add it to php code. If you write a wiki page, you write a wiki page and that's all to it.
Thanks!!
You're welcome.
You appear to need to find out what a CMS is, since it sounds like that's what you require, not a wiki engine. Adding PHP code or CGI in wiki pages and having it executed would be nothing short of a security vulnerability.
If you wish to customise the MediaWiki software, then that's a matter of editing the PHP scripts which run it.
