I'm setting up MediaWiki on a CentOS 4.5 (RHEL 4 clone) box. The default MySQL is 4.1. This is horrible, but is installable via yum (the box is firewalled such that I can't hit the centosplus repositories easily), which saves me the pain of RPMs.
But how long is it likely MediaWiki will support MySQL 4?
(We try to run what MySQL we have here on the latest 5.0, which we usually install from source or *maybe* RPMs. We prefer Postgres for our internal stuff, but I'd rather MySQL for MediaWiki 'cos it's the main platform, i.e. what Wikimedia runs. (Same reason I'm putting it on our Linux app server instead of continuing to battle Solaris 9.))

4.1? For the forseeable future.
4.0? Also for the forseeable future. We have yet to migrate off 4.0 ourselves.
Note that the biggest difference for our current usage is not at 5.0, but at 4.1, which is where all the character set stuff changes.
