All but ~500 out of ~22,000 users on my wiki have never made a single edit. Is there a good way to clean out these junk records? Is there an extension that puts e.g. a captcha on the user register page (not just edit)?

I'm assuming you're talking about ConfirmEdit. ConfirmEdit does provide captcha on the registration page.

No - at least I have not found such a way. And I'd like to delete some hundreds of these spammer entries.

"ConfirmEdit" has helped a long time against those spammers.
But since some weeks there's a robot which first gets an account and then changes the main page - very nasty.

Protect the main page? Install AbuseFilter and add a filter that automatically stops them and block the account if their first edit is to edit the main page?

Seems so. I've just looked into the source code of the page: it's simple to find the captcha question and to calculate the answer.

I recommed to choose reCaptcha. It's barrier-free and really nasty to crack by bots. (Well, it's google and I respect everybody saying you should not rely on this service)

From the url you provided, I see you are using the math captcha. That is very simple. You should switch to FancyCaptcha.

Sorry - my wiki uses "simpleCatcha". I've just tried to change to "MathCatcha" following the instructions in
path/to/wiki/extensions/ConfirmEdit/MathCaptcha
(version from May 6 2011)
and edited only "LocalSettings.php". That led to the error message

There's that. Also when deleting a user there's also likely to be log entries (newuserlog) as well, so just deleting the user record is insufficient.
