Basically, it's done with some URL rewriting, that is, a bit of
regular expression matching and replacement on the request URL.
MediaWiki can then be instructed to output normal article links in a
special manner, thus making it all appear seamless.
There are, however, hundreds of millions of different configurations
out there, and many different implementations of this, so you'll
likely find different snippets about it here and there.
