I set "$wgSend404Code = false;" in LocalSettings.php (Default value:true), then the issue has gone away.

My guess is that your host injects text into pages that include a 404 response. (This is similar to https://bugzilla.wikimedia.org/show_bug.cgi?id=18270 )
