When I view this page http://olr.freemason.com/wiki/index.php?title=Mackey:_An_American_Landmark logged in, I see all the footnotes correctly.
However when not logged in, all note references are numbered [1] and none of the notes themselves display.
Ideas?

I am only using footnotes on this set of pages, since the wiki is new. I see them fine (both numbers and notes text) when I'm logged in as sysop, which is why I hadn't realized there was a problem till now.

What happens if you log in as a non-sysop user? And what happens if you create a new page with some refs?

For your site, I see the citation markers but not the endnotes for both of the links you provided. It looks like you're seeing this known bug:

The form of extension I am using doesn't use template pages - it's only broken when the user isn't logged on.
