I'm newbie with semantic form. I know how to create simple form with multiselection and/or some fields type and/or more templates. I would like to understand how I could to create a dynamic table (like http://genomics.cribi.unipd.it/~bilardi/dynamic/) into a form for creating page with a static table with results.

I read http://www.mediawiki.org/wiki/Javascript so that I create my MediaWiki:Dynamic.js but I don't understand if it is right way and how/where to recall javascript function into form page.

Can you help me about it?
