I've seen these "180 buy Viagra"-type accounts on wikis with math captchas, fancy captchas, and re-captcha. And of course on wikis that aren't using the ConfirmEdit extension.

I threw together a little plugin for ConfirmEdit that uses questions instead of word-image CAPTCHAs: http://www.mediawiki.org/wiki/Extension:QuestyCaptcha
Humans can still solve them, of course, but a person working in an Indian CAPTCHA-cracking farm might have more trouble answering a question specific to your site than just reading a word in an image. If you try it out, let me know how well it works. :)
