[Mediawiki-l] Any reason why a page would appear "orphaned", even though it's inderectly reachable from the main page?

Just like the title says, we're getting pages in the "orphaned" list
that are no more that a few clicks away from the main page of our site.
Anyone care to hazard a guess why?
