* Operating system * MySQL version * PHP version * the exact procedure you used to backup and restore <- very important! * whether you've checked MySQL logs <- very important * whether you've looked up documentation on mysql.com for how to check for and repair damaged tables <- extremely important * whether you've checked web server or PHP debug logs * whether you've looked up documentation on php.net for how to enable logging
