Not unless you've installed and configured it.
It's installed and works for regular pages.. does it not work for userpages and usernames by default?
I haven't used Title Blacklist much, but from looking at the documentation, it seems like you may need to edit MediaWiki:Titleblacklist-forbidden-new-account.

There is one usability issue: If the wiki is set to always require confirmation, it is still only show the captcha after the first save try. The captcha should be shown right in the edit page before saving if it is necessary anyways, thus avoiding an additional step in order to save the page.
