Stripped down to its essential bits (from https://en.wikipedia.org/wiki/Canonical_form) what you're looking for is the "standard way of presenting [an] object". The object in question here is the URL for any page on your site.
So, your canonical path for the Main_Page would be:
http://www.mysite.us/w/Main_Page

Should I change $wgServer = "http://mysite.us"; to $wgServer = "http://www.mysite.us"; and get rid of the redirect at the beginning? Do something else?
If your site is generally accessible on the Internet, it wouldn't hurt to use the actual domain name. We might spot something that could help us solve your problem that way.

So, yes, you should change $wgServer to "http://www.mysite.us".
