A vandal nearly 3 years ago that only posted for 2 days with no similarity in the messages. I think the use of the name "Pussy man" is a coincidence, it's not a particularly unusual name for a vandal/troll to choose.

I am fairly sure it is a coincidence. I have blocked a couple of user names with the p word . It's just I found it hard to believe that no body caught it.

Caught what? You mean that the vandal was never blocked? They stopped vandalising on their own, so I suppose there was no need. Warnings should have been issued, at least, but it's not unusual for people not to (I've forgotten myself on occasion).

I forget too. If I remember correctly, people didn't warn users as much back then. We didn't have WP:UAA http://en.wikipedia.org/wiki/Wikipedia:UAA then. There wasn't as many admins either.

On the contrary I'd argue that the extra layer of bureaucracy for dealing with offensive user-names means that such users are more likely to be disregarded than blocked.
Of course if you do pursue it to the final round you can roll your eyes at the (very imaginative) explanations given by those who frequent user-name RFCs:
