No, it's no fun at all, but in the German wikipedia we decided to do exactly this (Scipius was so nice to mail me the flags in a zip-file). Reasons: there is no possibility to control whether pictures are in use in other wikipedias. Pictures may be changed or deleted without a possibility to say: hey stop, we still need this picture!
Thus we need, say, the [[fr:]] image page to look at the [[nl:]] image page when it says whether the picture is being used.
Something like [[image:en:Image_US_flagmedium.png]]
It should be [[en:image:Image_US_flagmedium.png]], but otherwise you're right.
BTW: this is a subject which is definitely of concern for the international list. Or is wikipedia-l the international list now?
