Nice survey!! Just a few things to consider:
Question8: is it possible that people who particitpated in WLM 2011 started right away in october to make new photographs? So answer 2 might be: I started right after WLM 2011 up to september 2012.
Question 9: motivation might also be found in wanting to share images of heritage in my town, village, environment or because it is useful to document heritage. The answers sound a little 'egocentric' to me, a little to much form point of view of Wikipedia instead of the point of view of the uploader.

Good survey, I especially liked how you reassured people as to what you'd use their information for and that you only asked them their year of birth. I hope you don't mind but I've forwarded this to the rest of the research committee and I hope that we will get more feedback from them.
Q9 needs an "It was fun!" option because previous surveys have found this to be one of the most common motives of our volunteers. I'd also suggest an option of "I'm proud of my photos and wanted others to see and use them", plus Els' suggestion which maybe could be worded as "I wanted to document the heritage of my area".
May I suggest you add questions to find out whether people have used these photos in Wikipedia, whether they have used the Quality featured process and what other topic areas they might be interested in participating in. You could word these as
Question W Have you added any of the photos you took to Wikipedia?
1 Yes 2 No, the monument I photographed doesn't have an article on Wikipedia 3 No, the Monuments I photographed already have pictures in their Wikipedia article. 4 No, I'm not sure how to do that. 5 No, not yet but I might in future.
Question X Have you submitted any photographs for consideration as Featured or Quality images in Wikimedia Commons?
1 Yes 2 No, I don't know about those processes 3 No, My camera isn't good enough to create that quality of photograph 4 No, I don't think any of my photographs are up to those standards 5 No, not yet but I might in future.
Question Y Will you take part in Wikimedia loves Monuments in September 2013?

@Els: I think that this is a good idea to change the second answer into the one you suggested, so I went ahead and did so (it now reads: "I took the pictures between October 2011 and August 2012").
I also agree with your other suggestion (and I think it was also brought up by Jane, too) of adding the willingness-to-share option, so we now have 5 possible answers for Q9 (+ an "Other" option).
While preparing this survey, I've been reading some great survey tips from a Harvard University researched, David S. Walonick, who suggested that you should not present people with more than 5 choices for an answer, because it makes the survey/questionnaire too long and boring for the respondents, and I would like to stick to that suggestion. (It was also he who suggested to include a confidentiality/anonimity policy that I wrote down in the introduction.)
I am not sure how you know that "It was fun!" was a major motive that people chose in our previous survey(s), because I can't remember the 2011 survey results ever being published publicly (due to us not having the manpower to work on them). Perhaps you were talking about different surveys that I don't know about?
I am also not sure if asking people questions about placing the images on Wikipedia or using the Quality images process actually fits in the scope of this survey; this is a hard-core Wikipedian's job, and I doubt that the new users we try to reach with this survey even know how to place an image into an article, or what a Quality image is.

I wasn't aware of previous surveys to WLM participants, I was thinking of previous surveys of Wikipedians and Wikimedians. One key finding has been that people volunteer because they enjoy doing so, and that it is the most important motive for our community.
As for survey length, I wouldn't be too concerned about the length as long as the questions are interesting and relevant to the participants, and crucially they don't have to put the survey on hold and ask someone else before answering a particular question (I didn't spot such problems in your survey but generally the longer a survey the greater the risk that someone asks such a question).

I'd split in October 2011-March 2012 and May-August 2012. The people who started photographing for WLM2012 in October should be a different bucket than those that noted that for their summer holidays.
It'd also be interesting to note when the photos were taken thinking on the contest, and when they were "available photos".

Talking about placing the images in Wikipedia may be ok. I'm much less convinced about QI. That's not a goal of the contest.
I really liked seeing this answer in last year survey: «Easy, Help, Prize, Other:I wanted to learn how to upload pictures to Wikipedia, as I have never done that before and this seemed like a good cause. :)»

Ah, looks like I forgot about this one, but I was mostly thinking about organised results (with graphs, etc.), not just about answers put into an Excel file; I can vaguely remember that there weren't too many volunteers to do that part of the job (which is understandable).
I would add a question about the way they uploaded their files:
Aren't we able to get this data without asking this question in the survey? I was under the impression that we can at least get a more-or-less correct number of UploadWizard uploads through the CatScan tool; I know it would also work for the VicunaUploader created by Yarl; not sure about Commonist and the old upload form.
Maybe even with a textarea for them to complain about our upload tools (in case they have errors).
I think we can use Q14 ("What was the main obstacle, if any, for you to participate in the competition? If you wish, you can elaborate in detail.") for that one.
Another question that I thought could be good was, "would you be willing to collborate organising WLM 2013 in your country?".
I think it was also suggested by Tamara on Etherpad today, so I guess the question has some merit (no offence Tamara!), but I am not entirely convinced we should use it. We already have contact information to Wikimedia user groups that organised WLM 2010, 2011, and 2012, and have also received contact information from a lot of people from countries that did not participate yet through the blog.

Could we add a statement at the top that there are no obligatory questions?

Just as a short remark: Most question suggestions are quite good... but we should really really limit the survey to about 15 questions like last year. Having a short survey proved to be very helpful in the number of responses (got good feedback on that). I'm already having fights with Tomasz about the 4 extra questions added so far - so please realize that if you want to add a question, another question will likely have to disappear. Many good questions were quite detailed, and with every question we should consider what we want to learn from that answer.

Yes this does look good! My only suggestion now is to trim some questions (sorry, it's hard for me to choose which ones) and to keep the italics, but do not use the light gray color and keep to black-on-white for increased readability. Jane 2012/10/11 Raimond Spekking raimond.spekking@gmail.com

Hi Platonides, thanks for your work on this, it looks great.
Just a couple of small details: I thought about having our suggestions ("Select all that apply, "You can leave that field empty", etc.) moved below the questions, and marked with a grey font (I see you use CSS for that, so it should be easy enough.)
I would also remove the <h2 /> for the Introduction part, and would appreciate it greatly if you could replace the Toolserver logo with our own (WLM). I would also suggest setting Vector as the default skin for the survey.
And a few more questions: will the survey automatically recognise the language of the user, or will we need to have a languages box at the top of the page? How about getting acccess to the source of the survey, and its results, where would those be stored?

Yes. It will pick the language prefered by your browser amongst the available translations. Besides, you have the full translation list at the sidebar.
How about getting acccess to the source of the survey, and its results, where would those be stored?
That part is not implemented. I see a couple of options: a mysql table or saving to a file.
