Could be a good idea for wikiproject folks to work on a RSS feed of recent changes (and recent new page creations) within a category and its subcats. Sounds like a thing that'd increate all our participation in WikiProjects in the first place - and resolve plenty of user retention issues.

I'd like all our medical articles to have a button at the top saying "email me when this article changes", so interested experts could easily adopt a few articles. (But we're getting a bit off-topic here.)

It's possible to do this with a single-purpose account and "Email me when a page or file on my watchlist is changed" (which was re-enabled on enwiki a few years back). Not the most straightforward process, but it's there; you could do it with a regular account, but you'd get swamped fast!
A better solution would be some kind of "prioritised watchlist", which has been proposed a few times but I'm not sure if anyone's ever worked on it.
