While they were creating their accounts, some of them had their initial suggested username taken, so they had to repeat it a couple of times until they finally found an available username. How would that work? On what basis would it make suggestions? Would it just tell you what the next available number to add onto the end is? I can't think of any other suggestions it could make...

I believe there was someone working some ajax (extension possibly?, maybe some core work?) to improve that on the sign up page, No idea what ever happened to that…

You can't use name if this one is unified login, even if this name not exist in local project. Special:ListUsers is only a local list. Probably most SULs exist in en Wikpedia, but not all.

This script gives a list of every account using a specified name on WMF wikis, and displays SUL data if accounts have been merged."
The catch though is that if there is not an exact match it just returns no account. Special:ListUsers shows most close matches.
It serves well enough for creation of an account on the wiki you're running it on. But there might be someone on another wiki using the same name, or one that is confusingly close.

Like the e-mail system.

We also have spoofing checks coding in the system which attempts to prevent creation of similar names. You run into that when you try to create an account. You have to override it at Special:UserLogin/signup in order to create an account that is close in form to an existing one.
This spoofing check could be run before the person actually tries to create the account then fails.

BTW - we have 1 link "Log in / create account" to the special page Special:UserLogin. It is not good . Option with 2 links: "Log in" or "Create account" will be better.
