Fwiw, I have been noticing some problems like that on en (probably images coming from Commons). In particular I noticed that the larger I made the thumbnail, the more likely it was to break. In one case, I couldn't get a thumbnail larger than or equal to 700px - but maybe that's a known hard limit?
