I don't think they need the language name (either in English or in the local language). If the project is approved, then it must have the interface translated, which would mean that it's already defined in Names.php.
- language code
- type of project (Wikipedia, Wikinews etc.)

- the name of the project namespace
- the name of the project talk namespace
It looks like from bug 14252 comment 7 https://bugzilla.wikimedia.org/show_bug.cgi?id=14252#c7 that we don't actually need the project talk namespace, if everything is properly defined in Messages*.php.
Anything else needed?
...and anything else "wanted in a perfect world"? (For example, a logo isn't technically *needed* but it's easier to have them setup and get everything working right away, in the initial bug request, than to try to track someone down later.)
