Having two HTTP response codes certainly looks strange, and could be
treated differently by different browsers. But I don't know whether
it's a PHP/IIS/configuration/MediaWiki bug.
If you're prepared to experiment, try changing line 111 in OutputPage.php from
header( "HTTP/1.0 304 Not Modified" );
to
header( "HTTP/1.1 304 Not Modified" );

You could try the latest MediaWiki version 1.4.4, as it includes a fix for IIS.
