Sounds like a parsing nightmare for an already somewhat disquieting parser, given the use of '' for italic, ''' for bold and ''''' for both together.
I've had a devil of a time getting some weird URLs to work, e.g. web.archive.org ones with http:// in the URL (workaround: leave it out, the page is just the same ;-).
Possibly something could be added to say "this is a URL, don't mess with it".

In fact, this does not work, try [http://www.example.com/%27%27foo'']. But I have to say I don't consider it to be the greatest problem of MediaWiki. :-)

1.3 is in maintenance mode and is receiving only security fixes.
Either backport the fix yourself or upgrade to 1.4.
