I've just committed a change that allows users with the permission key 'ipblock-exempt' to be exempted from ALL forms of IP blocks, including those which come from DNSBLs.
This permission is by default assigned to the Sysop group, although it can be, as usual, applied to any group using $wgGroupPermissions.

How can we on a wikimedia project enable this for a user? There aren't added or changed any Mediawiki messages in r18904.

Make them a sysop. ;) If you want all autoconfirmed users or something to be exempt, that strikes me as possibly a bad idea, but you can file a bug at bugs.wikimedia.org asking for it if you like. Since we don't yet have a flexible userrights system, no, you can't make a specific group for this and add only specific users to it.

Nope. There's not yet any interface to add such usergroups unless you're a steward, so no usergroups are added other than the standard set.
