Thanks to Aaron Schulz for debugging with help from Chris Steipp and Aude testing we fixed the issue.

We now have re-deployed 1.21wmf12 to the phase 1 and 2 wikis (see: https://www.mediawiki.org/wiki/MediaWiki_1.21/Roadmap )
