When creating thumbnails I've been successful at selecting the particular frame of a video to be shown as the thumbnail. Is there a way to do something similar, to show a particular page of a PDF as the thumbnail in an article?

Just add a "page" parameter to the file syntax: https://www.mediawiki.org/wiki/Extension:PdfHandler#Usage
