Maybe related: having both SemanticMediaWiki and Visual Editor installed will still reliably trigger this error.

Yes, that looks like it. Try applying the core EditPage and VE patches.

I don't know about new versions of SMW, but wikitech seems fine.

Looking at the 1.28.0 sources (core and VE) on my host, I believe those patches are already present.
