I just look at http://meta.wikimedia.org/wiki/Ultimate_Wiktionary_data_design to me it seem as if this project is talking about different database structuer?
Is this true? Could this be teh first time a mediawiki project have a different db structuer? Is there policy saying that all mediawiki projects must have same db structuer?

Yes.
There are a couple of configuration differences - sub-pages allowed in the main namespace on Wikibooks, custom namespaces in various places, Wiktionary doesn't capitalise the first letter of titles (so "Foo" is not the same as "foo"), etc, but these are all options available to anyone installing the MediaWiki software, and make no difference to the basic database structure. There are also one or two additional extensions installed - such as the <DynamicPageList> on Wikinews - these are also available to anyone who wants them, and are relatively superficial.
