I see this from time to time on the English wikipedia. I believe it happens when the wiki and/or my network are extremely bogged, such that the wikipedia server doesn't even manage to send the first bit of the HTTP response (the bit indicating that the response is HTML even though the URL doesn't end in ".html") before the server and/or my browser times out, and my browser decides that "index.php" is an unknown Content-Type which it might as well let me save.
This sort of problem might be more likely when you're logged in, because the wikipedia server is much more likely to have to rerender the page for you (with your own personalized buttons across the top) rather than tossing you one out of its (squid) cache.
I think I was seeing this more often a couple of weeks ago, when we were struggling with various too-complicated nested templates which were taking waaaaaaaayyyyy too long to render.
