Howdy,
The most likely cause is that the MySQL database server has not been running because of the way that the servers were started. That would also explain why there is no log from the database server.
The command-line "start.bat w mysql" should start the database server - that's the final parameter on the command-line. You may want to create a "startall.bat" file containing that line in the same folder as the supplied start.bat The supplied stop.bat shuts down both Apache & MySQL.
To see if MySQL is running, run the Windows Task Manager, and look for a process called "mysqld-opt.exe".
Alternatively, from the command-line run: netstat.exe -n -p tcp -a|find ":3306" If netstat shows a line with "LISTENING", it is running.
Apache can be used on the same machine as IIS, but as only one web-server can use port :80 at a time, you need to change the Apache port from :80 to e.g. :81 (in httpd.conf) and then use initial URLs like http://localhost:81/a/ and http://localhost:81/wi/
