It's a book, Tom. I suppose copyright in this case is the same as other traditional
book copyrights.
As for the other question, nowadays I've been rather more concerned about having to
pay the publisher in order to be edited than the other way round. I don't know if this
is also the case for books, but I find it alarming that this abominable practice has
turned to current practice in academic journals.
