I like it a lot, very professional.
A few suggestions that haven't been pointed out by my fellow friends:
* Add a []Don't care checkbox in 'Preferred Room', I personally don't care if I'm with other person or 7. * Under receipt : Mailing Address, the field should be a textbox to put the address not a textfield.
The rest, I think it looks really good, very professional, maybe if you template it I might be able to translate it to spanish (the final version) if you wish.
