* Why do you need such big amount of data? You should mark as little fields as possible as required in some way. There should also be a little privacy note - I'd never submit my passport number and birth date if not really needed and you should not make people to do this.
* What is ATM and how does it work? A link to Wikipedia should help ;-)
* "Preferred Wikimedia account" instead of "Wikimedia account" - I have many.
* Optional "organization" if person has an affiliation
