No. There will be a lot of lexemes that will not be used as labels for data items (just as there are many words/meanings on Wikionary with no Wikipedia article), and there will be some labels that are not lexemes (like most names of People and Places, and also perhaps common misspellings).

The proposal does not detail how lexemes interact with items in the UI and in search - that's still open, and that's exactly where the things you have been mentionen will have their place.
