1) You need to justify each use (there isn't a limit) with a fair use rationale. See [[WP:NFCC]] (type these into the to search bar, without the "[[" and "]]", and click go) and [[WP:FURG]].
2) The smaller the better. There might be a maximum; I'm not sure, but really, try and keep it as small as possible. Large album cover images can be tagged with {{non free reduce}}.

You can use it as many times as is absolutely necessary. There isn't a size limit, but for album covers, they should be as small as possible.
