The dutch language Wikipedia has solved this problem several years ago as follows;
Put somewhere an external contact form online that you can use to send an email to your Wikipedia. The OTRS-contact email address can be used. For EN is that info-nl@wikimedia.org And link from the page that the blocked users see to that contact form. This way users who are blocked can easy request for an unblock.
This system works very well since the end of 2002 for the Dutch wikipedia. We also put the sysop who has done the block in CC if we grant the unblock request so that sysop knows why you have done the unblock.
