enwiki has [[Wikipedia:Example requests for permission]]. I'm not sure
of something exists on commons also. People also ask questions
somewhat often about the licensing issues regarding uploading images
taken by their grandparents or other older family members. We explain
the issues and as far as I know it seems to go pretty well. I don't
know what the actual conversation is of course, and how informed the
grandparents are, but the uploader/grandchild seems to understand most
of the time.
I've contacted people I don't know, and it works well, but most of the
people I contact are usually at least somewhat aware of free licensing
and wikipedia. Contacting people for permission is pretty common I
think.
