_NOTOC_ prevents a TOC from being displayed at all. There is currently no way
to force a TOC to display minimized other than clicking "hide".

3 headings are needed before a TOC is generated.

Perhaps the problem is that you are creating too many headers? ;-) Properly
constructed small pages usually don't need more than a heading or two.
There was some initial talk about creating a _TOC_ tag that could then be
placed where the author saw best to place it (including in a left or
right-aligned table or div), but I don't think that was ever implemented
(can't test it right now since the database server is down).
