What about changes on commons.media.org?
e.g.
en.wikipedia.org/wiki/ArticleFoo uses
commons.media.org/wiki/File:Bar.jpg, when the image file is updated on
commons.media.org, will there be an "rc type=external" entry appears in
en's recentchange list?

That's right. When a change on Wikidata is made, it injects these
rctype="external" entries into the local wiki so that the Wikidata
changes can show up in the local Special:RecentChanges and
Special:Watchlist.
