We have a terrible lot of files (thats a good thing) and I believe not so many people patrolling the recent changes. While I have seen three complaints about vandalism that was there for more than 3 days this month. I believe we must try to keep Commons for the visitor so clean as possible
3 days is a lot for vandalism to something as high profile as Condoleezza Rice, but on a number of ocassions I've reverted obvious vandalism that was multiple months old on lower profile images.
As I said, things that get in past the immediate watchers tend to stick around on Commons.
