That seems pretty dodgy to me for a survey that is soliciting feedback about harassment incidents (aka "negative situations").

p.s. How do I know what I'm a "member" of? Is being on this list sufficient to make me a member of Wikimedia Boston / Boston Wikipedians?

Thanks for catching that, Tom! That's a major oversight, I've let them know. Obviously the survey as a whole + the long-form answers in particular should highlight that answers will be shared publicly, or should disallow public viewing.
On membership: There is a members list for the New England Wikimedians on Meta which you may join or not as you see fit: https://meta.wikimedia.org/wiki/New_England_Wikimedians/Members We share user group updates with this list; you can be a member of the group & not on this list, and vice versa.
