https://phabricator.wikimedia.org/T104086 is a bug report template for Android issues. The easiest way to file a new bug is to go there and click on the link to file a pre-populated Phab ticket. Most of the time the "regular" one is what you need. If you fill out bugs coming from OTRS or crash reports then use the second one.
You can also find this template in the last column of our Android workboard[1].

Toby,
This is on mobile-l. Did you have any other distribution list in mind?

Should we link from the app itself?

Hey Bernd,
How are you generating the links? Sounds like a great way to make the "template" cards actually super useful.
I would recommend putting those links in the project description ( https://phabricator.wikimedia.org/project/sprint/profile/489/) too, even though that risks making them obsolete if we are not careful.

If this question is about the URL parameters to prefill certain fields: https://www.mediawiki.org/wiki/Phabricator/Help#Passing_certain_values_in_th...

Joaquin,
What Andre said, except I didn't have that handy help reference he linked to. I saw an example somewhere. The reason why the links in above Phab ticket might look a bit cryptic at first is the URI encoding of the formatting of the template text. I just wrote the template as I wanted it then URI encoded it.
I've added a link to it to the project description. Thanks for the tip!
