sounds fair enough. Either solution would work, although a personal opinion box will most likely always be checked. I prefer not making it obligatory.

Sorry for being so late on this...
I'm actually thinking that making the field "organisation" obligatory may keep some people from answering the survey, as they haven't had time to "talk" about this with their chapter and probably won't want to engage their chapter in this.
Possible ideas: *not make that field obligatory *add a "personal opinion" check box which allows people to give their personal opinion *make the name field obligatory and state that answers to this survey engage the respondant and noone else.
Whatcha think?
