It looks like the issue was due to "I go to a nonexistent page". On beta labs - the new page templates has lots of images which caused the upload button to behave differently (it assumes the page does not need an image - maybe a bug?)

The other issues seem to be related to a bad rule for the MainPage. I'm pretty sure merging https://gerrit.wikimedia.org/r/109053 will make them all pass again!
